As a Life Plan Community or Continuing Care Retirement Community (CCRC), members at Peconic Landing have a variety of living options available to them, depending on the level of care required to ensure that their comfort and individual needs are being attended to.
For those residents who require assisted living, memory care, rehabilitation or skilled nursing services, Peconic Landing is designed to support those needs, backed by professional medical team on call 24-hours-a-day, 7-days-a-week. As members needs change within the community and they move through different levels of care, our team works with members, their families, and physicians to transition to the most appropriate care setting. Each member’s care is customized for their individual needs and preferences, and delivered around-the-clock by dedicated professionals focused on helping each patient live better.
Upon membership, our residents benefit from our award-winning care services at no additional cost. Members of the greater community are welcome to enjoy these services at a per-day rate, when available. The Bluffs for Short-Term Rehabilitation
For short-term rehabilitative care, The Bluffs offers a state-of-the-art physical and occupational therapy center with indoor and outdoor components. Individuals can enjoy one of 17 private rooms with access to community center amenities.
Harbor North for Assisted Living
The Harbors at Peconic Landing for memory support: Harbor North is made up of 26 one-bedroom apartment homes with care services provided when needed. Members retain the benefits of an independent lifestyle with peace of mind knowing that someone is there to assist when necessary.
Harbor South for Memory Support
The Harbors at Peconic Landing for assisted living care: Harbor South features 16 private memory support suites with shared neighborhood amenities, including an outdoor “discovery” garden. Each member benefits from an individual care plan designed to provide purposeful living with all of the comforts of home.
The Shores for Skilled Nursing
The Shores, known for its award-winning care, is a full-service skilled nursing center offering around-the-clock long-term care. With room for 43 members, our residents benefit from access to community living spaces, while living right down the hall from their loved ones and friends.
Respite Care for Memory Support
Harbor South features 16 private memory support suites that are available for short ‘respite’ stays so Long Island caregivers can get a break from their responsibilities to enjoy a vacation, catch up on work obligations, or simply take time to rest.
Home Health Services
Peconic Landing Home Health Services enables seniors to receive the necessary support they need in the comfort of home. Peconic Landing offers nursing, home health aid and companion care, as well as physical rehabilitation services.
